Health
Physical health
If you have reached the age of 18 and you move away from home, you will need to register with a GP and a dental practice. GP appointments remain free of charge but in most cased you will need to pay for your dental treatment.
Mental health
If you are still receiving support from the Child and Adolescent Mental Health Services (CAHMS) you will need to transition to adult mental health care. Mind have set out some guidance on how this works.

Adult Social Care
The local authority will support young people with special educational needs and disability to transition to adulthood. The Social Care Institute for Excellence have produced a series of videos in which young people explain the process and talk about their experiences.
Education
Young people with an Education Health Care Plan (EHCP) are able to stay in full time education up to the age of 25. Young people without an EHCP can apply to colleges for a bursary for their student fees as well as for support with transport.
Employment
There are a range of services available for young people living in the ASE region.
- Safe in Sussex provide support to disabled young adults through sessions, activities, trips and breaks that build independence, transferable life skills and social interaction equal to that of their non-disabled peers.
Housing Support
GOV.UK provide guidance on housing costs and universal credit.
